Remediation Planner Template
Use this template to turn a failed training behavior into a targeted recovery plan.
Purpose
- tell a trainer exactly what to assign after a failed drill, checkoff, or live-case simulation
- keep remediation focused on the failed behavior instead of restarting everything
- make re-entry criteria explicit
Scope
- state the brand and training set covered
- state whether the planner is for drill failure, checkoff failure, simulation failure, or all three
How to Use
- identify the failed behavior first
- assign one primary drill or simulation
- assign one or two supporting documents only
- define the pass condition for re-entry

Escalation for Repeated Failure
- define when the trainer should pause the path entirely
- define when the trainee must return to an earlier progression stage
